Your Family Health Team (FHT) or Community Health Centre (CHC) will be contacting you in priority sequence for appointment scheduling. If you're a member of one of these priority groups, but not a member of an FHC/CHC, please connect with your respective community organization (noted below) to inquire about setting up an appointment:
* Health care workers
* Seniors 75+
* Indigenous Adults 55+
* Chronic home care recipients
